trimestru<\/b><\/h5><p><strong>Confirmarea \u0219i localizarea sarcinii<\/strong><\/p><p><span style=\"font-weight: 400;\">Controlul ecografic din primul trimestru confirm\u0103 dac\u0103 sarcina este intrauterin\u0103 \u0219i dac\u0103 este viabil\u0103. Sarcina extrauterin\u0103 poate cre\u0219te nivelul hormonilor \u00een mod similar cu o sarcin\u0103 intrauterin\u0103, ceea ce poate duce la un rezultat pozitiv al testului de sarcin\u0103 f\u0103cut la domiciliu. La un nivel de beta-hCG \u00eentre 1500 \u0219i 3000 mlU\/mL, sarcina intrauterin\u0103 trebuie s\u0103 fie vizibil\u0103 la ecografia transvaginal\u0103. Un embrion \u0219i activitatea cardiac\u0103 intraembrionar\u0103 apar la aproximativ 6 s\u0103pt\u0103m\u00e2ni dup\u0103 ultimul ciclu menstrual al pacientei.<\/span><\/p><p><span style=\"font-weight: 400;\">Pentru relevarea sarcinii extrauterine se poate utiliza deopotriv\u0103 ecografia transvaginal\u0103 \u0219i cea transabdominal\u0103. Majoritatea sarcinilor extrauterine sunt \u00een trompele uterine \u0219i este necesar\u0103 monitorizare sau tratament pentru a preveni ruperea acestora.<\/span><\/p><p><strong>Cauzele s\u00e2nger\u0103rii<\/strong><\/p><p><span style=\"font-weight: 400;\">25 la sut\u0103 dintre femei au s\u00e2nger\u0103ri \u00een primul trimestru al sarcinii. Unele s\u00e2nger\u0103ri nu au leg\u0103tur\u0103 cu sarcina, dar pot fi semne ale unui avort. Prin ecografie intravaginal\u0103 se poate stabili cauza s\u00e2nger\u0103rii, astfel femeia gravid\u0103 poate beneficia de tratamentul adecvat.<\/span><\/p><p><span style=\"font-weight: 400;\">Imaginea ecografic\u0103 specific\u0103 a avortului precoce este lipsa embrionului sau a activit\u0103\u021bii cardiace intraembrionare \u00een sacul gesta\u021bional mai mare de 25 mm. Absen\u021ba embrionului sau a b\u0103t\u0103ilor inimii cu o frecven\u021b\u0103 sub 85 de b\u0103t\u0103i pe minut poate indica, de asemenea, pierderea precoce a sarcinii. Riscul de preeclampsie este mai mare datorit\u0103 modific\u0103rii indicelui de pulsatilitate a arterelor uterului, lucru care poate fi constatat cu ajutorul ecografiei Doppler, precum \u0219i datorit\u0103 nivelului anormal al anumitor proteine fetale (PIGF, PAPP-A). \u00cen cazul unui risc major \u0219ansele de apari\u021bie timpurie a preeclampsiei pot fi diminuate \u2013 potrivit rezolu\u021biei WHO \u2013 prin \u00eenceperea administr\u0103rii unei doze sc\u0103zute de aspirin\u0103 (75 mg acid acetilsalicilic\/zi) \u00eenainte de s\u0103pt\u0103m\u00e2na a 16-a a sarcinii. \u00cen plus, monitorizarea regulat\u0103 a tensiunii arteriale \u0219i momentul adecvat al na\u0219terii sunt de mare importan\u021b\u0103 \u00een toate sarcinile.<\/span><\/p><p><strong>Malforma\u021bii congenitale \u0219i sindroame genetice<\/strong><\/p><p><span style=\"font-weight: 400;\">Anumite malforma\u021bii congenitale pot cre\u0219te riscul decesului fetal sau pot necesita luarea de m\u0103suri imediate dup\u0103 na\u0219tere. Faptul c\u0103 pot fi identificate \u0219i monitorizate malforma\u021biile sau a sindroamele genetice ofer\u0103 posibilitatea unei \u00eengrijiri mai performante pentru p\u0103rin\u021bi \u0219i pentru copil deopotriv\u0103.<\/span><\/p><p><span style=\"font-weight: 400;\">\u00cen primul trimestru, adesea, este prematur\u0103 identificarea malforma\u021biilor congenitale, dar unele anomalii cardiace structurale pot fi vizibile pe imagini ecografice de \u00eenalt\u0103 rezolu\u021bie.<\/span><\/p><p><span style=\"font-weight: 400;\">Ecografia ajut\u0103, de asemenea, medicii s\u0103 vizualizeze semnele anumitor anomalii cromozomiale sau sindroame genetice, \u00een special atunci c\u00e2nd este combinat\u0103 cu analize de s\u00e2nge.<\/span><\/p><h5><b>Trimestrul al doilea<\/b><\/h5><p><span style=\"font-weight: 400;\">\u00cen s\u0103pt\u0103m\u00e2nile 19 \u0219i 20 f\u0103tul este deja destul de mare pentru examinarea detaliat\u0103 a organelor. Examin\u0103m cerebelul, craniul, fa\u021ba, coloana vertebral\u0103, inima, stomacul, intestinele, rinichii, vezica urinar\u0103 \u0219i membrele copilului. De asemenea m\u0103sur\u0103m m\u0103rimea copilului, evalu\u0103m locul placentei.<\/span><\/p><p><span style=\"font-weight: 400;\">Lichidul amniotic este evaluat la fiecare ecografie cu ajutorul indicelui de lichid amniotic (AFI) sau cu cel mai ad\u00e2nc buzunar vertical (SDP). Valorile prea mari sau prea sc\u0103zute pot indica anomalii de dezvoltare sau alte afec\u021biuni patologice cauzate de placent\u0103 sau cordonul ombilical. Valorile AFI peste 24 cm sau SDP peste 8 cm sunt semne de polihidramnion, iar valorile AFI sub 5 cm sau SDP sub 2 cm semnaleaz\u0103 oligohidramnionul \u2013 potrivit defini\u021biei clinice publicate de StatPearls. \u00cen aceste situa\u021bii ecografia face posibil\u0103 identificarea \u0219i interven\u021bia timpurie.<\/span><\/p><h5>\u00a0<\/h5><h5><b>Examinare preventiv\u0103 \u00een al treilea trimestru<\/b><\/h5><p><span style=\"font-weight: 400;\">Ecografia efectuat\u0103 \u00een a 30-a s\u0103pt\u0103m\u00e2n\u0103 are drept scop monitorizarea dezvolt\u0103rii \u0219i aprovizion\u0103rii cu oxigen \u0219i nutrien\u021bi a f\u0103tului. Evalu\u0103m dimensiunile corpului (date biometrice), circula\u021bia ombilical\u0103, cantitatea lichidului amniotic \u0219i placenta. Totodat\u0103 efectu\u0103m \u0219i un control al unor anomalii cardiace care la evaluarea din s\u0103pt\u0103m\u00e2na 20 nu sunt vizibile, cum ar fi stenoza aortic\u0103, defectele minore ale septului atrial \u0219i ventricular. Acest procedeu const\u0103 din analiza micilor fragmente de ADN aflate \u00een circuitul sanguin al femeii \u00eens\u0103rcinate. Spre deosebire de majoritatea ADN-urilor care se afl\u0103 \u00een nucleul celulei, aceste fragmente circul\u0103 liber \u00een s\u00e2nge \u0219i nu se g\u0103sesc \u00een celule, de aceea se numesc ADN-uri extracelulare (cfDNA).
